Abstract

Symmetric facial appearance is an important determinant of facial aesthetics. Bimaxillary orthognathic procedures can be used to restore facial symmetry in complex facial deformities.

We present a case of 30 year old female nurse who presented with obvious facial asymmetry involving both the maxilla and mandible bilaterally. The right maxilla and mandible were hypoplastic while the left side was hypertrophic. There was an associated maxillary occlusal canting. The patient underwent a bimaxillary orthognathic surgical procedure. Facial symmetry was achieved intra-operatively and was maintained post-operatively. A horizontal occlusal plane was achieved with an acceptable occlusion. Bimaxillary orthognathic surgery is an option for facial reconstruction in selected cases with skeletal discrepancies of both jaws. Adequate preoperative planning, investigations and preparation are imperative for successful treatment. The indications, patient preparation and procedures for bimaxillary orthognathic surgery were highlighted.
